First make sure that the port the modem is on matches the port it is listed on in the chooser. If not, remedy that
Make sure that Appletalk is turned off (In the chooser - reboot after changing)
Close any fax software. Try removing any Fax related extensions temporarily
If that doesn't work, try changing the port the modem is on (both physically and in the chooser)
